step 1
go to the ACP and install the addon as an archive

step 2
go to the feedback system menu in acp, and click "Add Feedback Forum"

here you will be presented with a list of options specifically for the forum you just created.

required feedback credits - how many credits required to post a thread in this forum

required credits for bump - how many credits need to bump a thread in this forum, I usually set this number higher than the required feedback credits, it promotes more activity, and stops users from spamming the forum by bumping their thread unless they have enough credits and are contributing.

penalty - if enabled there will be a feedback penalty for low effort post.

The strikes are accumulated every time a user gets their feedback deleted in a feedback forum, after so many strikes they will not be able to post in that forum again.

How to test

After you have added the forum, everything is now working.

If you want to test, you can create a bunch of dummy accounts to post feedback and give feedback.

Make sure moderators are checking the this is a feedback thread option.

The feedback system will not be enabled if there is not enough threads that need feedback.

Make sure that your mods and admins are clicking the this is a feedback thread option, if they need to make a regular mod thread they do not have to click this option.
 
If you go to ACP and manage feedback forums and click on your feedback forum there will be options specifically for that forum, you can edit the required number of credits to post threads, and bump threads, and also the required characters to post in the thread, by default it is set to 250 to encourage detailed feedback but if you do not need detailed feedback you can set this a lot lower or set it to 0.
 
If a user has provided low quality feedback or a troll comes in the thread to cause problems and you have to delete their post, each time you delete their post they will accumulate strikes, and after so many strikes they will not be able to post in the thread again.
